Forms & Documents

The following forms are required for students receiving a Howes Summer Grant to conduct an Independent Study Project.

• Medical Release and Authorization Form - a parent or guardian signature is required even for students 18 years of age or older (a faxed copy or scanned copy with the parent/guardian signature is acceptable)
• For those traveling internationally, 2 copies of the passport picture and signature pages are required. The passport should be valid for a full 6 months from the date of re-entry in the United States after completion of the program.
• Supplemental Authorization Waiver for Students Traveling to Countries Under US State Department Travel Warning.  This is specific only to those students participating in University endorsed activities in countries under U.S. State Department designated Travel Warnings.
