Forms & Documents

The following forms are required for students conducting an internship during Winter Term.

• Medical Release and Authorization Form - a parent or guardian signature is required even for students 18 years of age or older (a faxed copy or scanned copy with the parent/guardian signature is acceptable)
• Code of Conduct Form
• For those traveling internationally, 2 copies of the passport picture and signature pages are required. The passport should be valid for a full 6 months from the date of re-entry in the United States after completion of the program.
• Supplemental Authorization Waiver for Students Traveling to Countries Under US State Department Travel Warning For students participating in Winter Term activities in countries under U.S. State Department designated Travel Warnings.
