Off-Campus Faculty-Led Required Forms and Documents
Students participating in all off campus programming through DePauw are required to complete the forms below.
Forms and Documents
Required
- Code of Conduct: Students are expected to adhere to the Code of Conduct as stated by the Student Life Handbook throughout their time off campus.
- Medical Form: Students are encouraged to self-disclose any medical history, dietary restrictions, and medications to faculty/staff leaders. Any information provided can prove vital in an emergency situation, or when planning a group meals. Information provided will remain confidential.
- Medical Release and Authorization Form: For all students participating in an off-campus programs, regardless of age.
Supplemental
- Supplemental Authorization Waiver for Students Traveling to Countries Under US State Department Travel Warning: For students participating in Winter Term activities in countries under U.S. State Department designated Travel Warnings.
- Alternate Travel Arrangements Form: Only for students intending to travel separately from the group and with permission granted from the faculty leader.
Program Specific Information
Winter Term Off-Campus Faculty-Led Programs
Forms are also made available on the Moodle Pre-departure Orientation site. All students on Off-Campus Faculty-Led Winter Term programs are enrolled in this orientation site, and are required to complete the orientation prior to Winter Term.
All forms are due by September 30th, prior to the Winter Term.
