Steps for Receiving ADA Accommodations

 
Step 2: Documentation

Identification

Student submits completed Self Identifcation Disability Information Request form to ADA coordinator:
  • by July 15 for orientation accommodations--e.g., placement testing and early information about Academic Support programming
  • or within first two weeks of the semester or within one week of the receipt of appropriate documentation for accommodations during the semester
  • several weeks before the beginning of the semester for textbooks on tape or CD
Step 4: Implement Accommodations

Documentation

After receiving the Self Identifcation Disability Information Request form, the ADA coordinator provides: 
  • a letter requesting documentation and information regarding appropriate documentation
  • a letter confirming receipt of documentation

Appropriate Documentation:

  • is provided by an appropriately licensed professional
  • is current
  • identifies the condition as a disability which significantly impairs a major area of life functions
  • specifically describes the effects of the disability on learning and participation in university activities
Step 3: Determine Accommodations

Determine Accommodations

Once the documentation has been received, the student schedules a meeting with the ADA coordinator to discuss appropriate, reasonable accommodations. The ADA coordinator may consult with the professional staff at the University to verify disability and develop recommendations for accommodations.
Step 1: Identification

Implement Accommodations

If the student chooses to implement any or all of the accommodations, he/she provides permission for the ADA coordinator to notify appropriate faculty members and staff.

The ADA coordinator sends a memo to appropriate faculty and staff stating that the student has a disability and that the following reasonable accommodations are required to allow the student equal access to academic and extracurricular programs. The memo may include specific recommendations about how to implement the accommodations.

The student meets with appropriate faculty and staff to discuss specifically how an accommodation will be implemented. This meeting should take place within the first two weeks of the semester or within one week of receiving the disability notification letter in order to implement the accommodations in a timely fashion.
