Information and Forms for DePauw Student-Athletes Taking a Prescribed Medication Containing a Substance Banned By the NCAA

• General Information on Submitting Documentation to DPU Sports Medicine for all Categories of Banned Substances

               For Student-Athletes Taking ADHD Medication

• NCAA Guidelines Regarding Medical Reporting for Student-Athletes with ADHD Taking Prescribed Stimulants (reference for Athlete and Physician)
• NCAA Medical Exception Documentation Reporting Form to Support the Diagnosis of ADHD and Treatment with Banned Stimulant Medication (initial documentation to be submitted to DePauw Sports Medicine) 
• Annual Medical Follow-Up Documentation Form for Student-Athletes Taking ADHD Medication with NCAA Banned Stimulant Medication (after initial documentation is submitted to DePauw Sports Medicine, submit this follow-up documentation form annually)
