The Academic Credential (Acad Credential) form is a single-row form used to enter any of the client's previous college information. This form is also used to enter the client's parent's academic career. There can be more then one Acad Credential form linked to a client.
The client's or parent's Person form must be created or navigated to in order to open the client's Acad Credential form.
- Acad Credential. Use the Person Search link located at the top of the CIS forms window to open the client's Person form. If the client has not been entered in the Person form, then create the client's Person form before continuing. Select Acad Credential from the drop-down list in the toolbar. Finally click on the GO button to open the client's Acad Credential form.
- New. A new Acad Credential form can be opened by clicking on the New link on an existing Acad Credential form. This form will be linked to the client's Person form. Therefore a client can have several Acad Credential forms.
- Next|Prev. The Next and Prev links allow for selecting the desired HS Credential form when there are more than one form linked to the client.
The following describes the fields for entering the client's academic credentials information. If the client attended more than one college or other institution, use the New link to create another form.
- College/Institution. Select, from the drop-down list, the name of the college or institution that the client previously attended.
Attention: This is a required field.

- Credential Type. Select, from the drop-down list, the type of credential received from this college/institution.

- Date Credential Conferred. If applicable, enter the client's graduation date.
Note: See LOV (Calendar) and Eraser.
- Credential Description. Select, from the drop-down list, the description of the credential conferred.
- Date Attended From. Enter the date the client began attending the college/institution.
- Date Attended To. Enter the date that the client stopped attending the college/institution.
- GPA. If applicable, enter the client's GPA achieved at the college/institution.
- Comments. Enter any additional comments relevant to the client's academic credentials.

- Add Record. Click on Add Record when all of the fields are correct.
