AD Info Form

The AD Info Form is a single-row form used to enter the client's admission-specific information. 

 

Navigating to Form

The client's Person form must be created or navigated to in order to open the client's AD Info form.

 

Adding Information

The following describes the fields for entering the client's admission information.

 

  1. Officer.  Select the DePauw officer from the drop-down list or use the LOV person search.

    Note:   See Navigating Drop-Down Lists, LOV and Eraser.

     
  2. Officer Locked.  Select Yes if the officer is locked. The officer will no longer be updated even if the student's information changes.

    Note: See Navigating Radio Buttons.

     
  3. Merit Index.  Enter the Merit Index value.

    Attention: This field can only accept numeric values. 

     
  4. Merit Locked.  Select Yes if the Merit is locked. The merit index will no longer be updated even if the student submit higher credentials.

     
  5. FA Filer.  Select Yes if the student is planning to apply for financial aid.

     
  6. Travel Award Amount.  Enter in the amount awarded to pay for the student's travel costs for a campus visit to DePauw.

    Attention: This field can only accept numeric values.  Do not enter the dollar sign.

     
  7. Wait List Vote.  Select the Wait List Vote decided by the Wait List Committee from the drop-down list.

     
  8. ACT DPU Priority. Enter the number corresponding to the priority the student selected for DePauw on the ACT. For example, enter 1 if DePauw was the student's first choice to receive the scores.

     
  9. Intended School.  Select the client's intended school at DePauw from the drop-down list.

     
  10. National Merit.  Select Yes if applies.

     
  11. Potential Leader.  Select Yes if applies.

     
  12. Potential Parent Committee.  Select Yes if applies.

     
  13. DePauw Institute.  Select Yes if the student would benefit from attending DePauw Institute.

     
  14. Exceptional In Service.  Select Yes if applies.

     
  15. Alcohol Awareness.  Select Yes if the student has taken a course or has been involved in an activity reflecting alcohol awareness, such as Students Against Drunk Driving.

     
  16. Included In Counts.  Select Yes if the student is part of the current Admission pool.

     
  17. Primary Academic Interest.  Select the client's primary academic interest from the drop-down list.

     
  18. Secondary Academic Interest.  Select the client's secondary academic interest from the drop-down list.

     
  19. Add Record/Save Changes.  Click on Add Record/Save Changes button when all of the fields are populated with the desired information.
