Application Info Form

The Application Info form is a multi-row form used to record the client's application parts received and the related decisions. 

Attention: The client must have started an application before the notes and decision can be recorded.

 

Navigating to Form

The client's Person form must be created or navigated to in order to open the client's Activity form.

 

Adding Information

The form is divided into three main parts.  The first part is for recording the application type and decision type information.  The second part is for recording the decisions relative to the application.  The third section is for recording the application parts received.  

 

Application and Decision Types

Record the application types and decisions type on this part of the form.

  1. Last Application Type.  Select, from the drop-down list, if the client is applying for the College of Liberal Arts or the School of Music.

  2. App Packet Completion Date.  Enter the date when the last piece of the application packet was received.

    Note: See LOV (Calendar) and Eraser.

     
  3. Last Notification Type.  Select, from the drop-down list, the last type of notification given by the client.

     
  4. App Packet Load Completion Data. Enter the date when the last piece of the application packet was recorded in the administrative data system.

 

Decisions Made

Record the decisions made by DePauw and by the client regarding the client's application.
 

  1. Last Admission Decision.  Using the radio buttons, Select the last admission decision DePauw made concerning the clients application.

    Note: See Navigating Radio Buttons.

     
  2. Decision Date.  Enter the date the last admission decision was made.

     
  3. Notify Date.  Enter the date that the client was notified of the admission decision.

     
  4. Last Student Decision.  Select, from the drop-down list, the last decision made by the client.

     
  5. Decision Date.  Enter the date that the client notified DePauw of their admission decision.

     
  6. Other College.  Enter the name of the college the student chose to attended other than DePauw.

     

Application Part

The Application Part of the form records the date and receipt notes regarding the various parts of the application.  This section of the form is multi-row.  When one record is saved that is not Scores, the following parts of the application packet are added by default.

Each application part will have the same types of fields for data entry.
 

  1. Date Received.  Enter the date the application part was received.

     
  2. Receipt Notes.  Enter the applicable notes for this part of the application. 

    Note:
    This text entry  field will allow 255 characters.

     
  3. Waiver.  Select Yes if the waiver applies otherwise leave at No.

     
  4. Red X.  Select the red X to delete the entry.

    Attention:
     See  Red X (Delete).

     
  5. Add Record/Save Changes.  Click on Add Record when all of the fields are correct.

     
  6. Don't Save.  Click on Don't Save to not record the information

Note: Clicking on Don't Save will close the form without saving the information and will open the previous form.
