Concern Form

The Concern form is a single-row form used to record any significant concerns that the client's advisor may have concerning the client's personal or academic performance at DePauw.  Remediation can be offered to the client to ensure their success.  These concerns are confidential and only selected departments will have access to the form.

 

Navigating to Form

The client's Person form must be created or navigated to in order to open the client's Concern form.

 

Adding Information

The following describes the fields for entering concerns regarding the client.  If there is more than one concern, use the New link to create another form.

  1. Description  Enter the concern relevant to the client's personal or academic performance and the possible remediation to offer the client.

    Suggestion:  See Pencil.

     
  2. Security Access.  Select, from the drop-down list, the departments that will have access to this record.

    Attention: 
    The concerns are typically confidential, ensure that only the necessary departments have access to this record.

    Suggestion:  Press the  Ctrl key and left click the mouse to select more than one department.

     
  3. Add Record.  Click on Add Record when all of the fields are correct.
