The High School Visit Card form is a combination of Client Information Services forms. It is used to load client information collected on High School visit cards. New students can be loaded here, or students who are already loaded can be updated on this form.
The following fields are required to save the record.
Last Name. Enter the client's last name.
First Name. Enter the client's first name.
Address 1. Enter the client's street address.
City. Enter the client's home city.
State. Select the client's home state.
Zip. Enter the client's home zip code.
Year of Graduation. Select the client's Year of Graduation.
High School Name. Enter the CEEB code into the Lookup or Click the Flashlight to select the client's High School.
Intended School. Select the client's intended school from the menu.
The following fields should be entered together.
GPA and GPA Source. Enter the student's GPA as well as the Scale and Source (Self-reported).
Class Rank out of Size and Rank Source. Enter the student's Class Rank as well as the Size and Source (Self-reported).
Note: If the student has official scores in the database, then it is not necessary to load the self-reported scores. The official scores will be displayed as read-only.
Note: If the student does not have official scores, then the scores can be loaded and their source is automatically set to Self-Reported.
Note: Two instruments, two previous extracurricular activities, and two sports can be entered on the HS Visit Card form. If the client has already entered instruments, activities, or sports, then the menus will show those entries.
If the student does not have a record in the database, then the primary source will be set to HS Visit - Officer. Otherwise, a secondary source will be added.
