The Organization form is used for entering the information of organizations that are linked to DePauw or to clients. Organizations include, high schools, other learning institutions, businesses, etc. This form will also allow navigating to other forms that are linked to the selected organization.
Use the Organization Search link at the top of most CIS forms to search and open a desired organization's form.
A new Organization form can be opened by clicking on the Maintain HS Info link under Admission on the e-Services page or by selecting the New link at the upper left of the navigation bar of an Organization form.
The Organization toolbar is used to navigate to forms that are linked to the organization.
- Search. The Search link will open the Organization search LOV.

- New. The New link will open a new Organization form.
- Organization. The Organization link will open the Organization form.
- Address. The Address link will open the organization's Address form.

- Digicomm. The Digicomm link will open the organization's Digicomm form.

- Relationship. The Relationship link will open a screen showing the relationships linked to the organization. This information includes the DPU Person ID number, name, business title and relationship type.
The screen will also list the students currently attending DePauw that are linked to the organization. It will list their last name, first name, middle initial and year attending. The Relationship screen will not accept any input of data. This screen simply shows the relationships that have already been linked to this organization.
Suggestion: Clicking on any of the names listed on the Relationship screen will open the selected name's Person form.
- HS Info. The HS Info link will open the organization's HS Info form.
- Visit Evaluation. The Visit Evaluation link will open the organization's Visit Evaluation form.
When a form is in the Update mode, the name and Organization ID number will be displayed below the toolbar. All of the toolbar links will open the associated form that is linked to the displayed organization.
Enter the applicable information in the following fields.
- Parent Organization. If the organization is part of a larger conglomerate, use the LOV to enter the name of the Parent Organization.
Note: See LOV(Search) and Eraser. An item must be selected in order to use the Camera snapshot.
- Organization Number. Enter the Standard Industry Number for the organization. For high schools, this is the CEEB code.
- Organization Name. Enter the name of the organization.

- Organization Type. Select, from the drop-down menu the type of organization.

- Standard Industry Code. Select from the drop-down menu the U.S. Census Bureau Standard Industry Code for the organization.
- DPU Rank Number. Enter the DePauw Rank number for the organization.
- DPU Contact Person. Enter the name of the contact person at DePauw using the Person Search LOV.
- Comments. Enter in any comments that are relevant for the organization.

- Date From. This displays the starting date that the Organization form is relevant. Normally this is the date the form was created for the organization.
- Date To. This shows the date that the form is good to.
- Add Record/Save Changes. Click on Add Record/Save Changes button when all of the fields are populated with the desired information.
