Special Program Form

The Program form is a single-row form used to record the special programs the client may be recruited for.  This form will be used to record: if an interview was granted, who interviewed the client if accepted and the decisions made.  The client may have more than one Program form.

 

Navigating to Form

The client's Person form must be created or navigated to in order to open the client's Program form.

 

Adding Information

The following describes the fields for entering the client's program information.  If the client has more than one special program to record, use the New link to create another form.

  1. Program.  Select the special program, from the drop-down list, the client may be recruited for.

  2. Date Last Application.  Enter the date of the last application the client submitted.

    Note:  See LOV (Calendar).
     
  3. Invited To Interview.  Select Yes if the client was invited to the interview, otherwise select No.

    Note:  See Navigating Radio Buttons.
     
  4. Interview Accept.  Select Yes if the client was invited to an interview and accepted the interview.  Select No if the client was invited to an interview and did not accept the interview.  Leave blank if the client was not invited to an interview.

     
  5. Date Last Interview.  Enter the date of the last interview.

     
  6. Regular Interviewer.  Select the name of the interviewer from the drop-down list.

    Suggestion:
      If the name of the interviewer is not on the drop-down list, then use the CIS Person LOV to select the interviewer.  See LOV.
     
  1. Interview Score.  Enter the score that the client received on their interview.

     
  2. Leadership Score.  Enter the score that the client received for leadership.

     
  3. Program Admission Decision.  Enter, from the drop-down list, the program admission decision.

     
  4. Date Notified.  Enter the date that the client was notified concerning the program admission decision.

     
  5. Student Decision.  Select, from the drop-down list the client's decision concerning the program.

     
  6. Notes.  Enter any notes relevant to the client's interview and decisions concerning program.

    Suggestion:  See Pencil.

     
  7. Date From.  Enter the date that this record is applicable.

    Note:  The current date is the default

     
  8. Date To.  Enter the date that the record is good to.  Normally leave the default date.

     
  9. Add Record/Save Changes.  Click on Add Record/Save Changes button when all of the fields are populated with the desired information.
