The Relationship form is a multi-row form that allows multiple records to be entered. This form is used for linking the client to his or her parents and siblings. The client can also be linked to other students as well as organizations.
Attention: Parents and Siblings must be entered on the Person form before they can be linked on the Relationship form. Any organizations must be entered on an Organization form before the client can be linked on the Relationship form.
Suggestion: The camera link, next to the Relationship Person field, will pop a new window with the Person form and the individual selected in the LOV. Use this link to quickly update the client's information, for example if the parents' address has changed. Close out the Person form window when finished to continue with the Relationship form. See Camera.
The client's Person form must be created or navigated to in order to open the client's Relationship form.
- Relationship. Use the Person Search link located at the top of the CIS form window to open the client's Person form. If the client has not been entered in the Person form, then create the client's Person form before continuing. Next click on the Relationship link in the toolbar.
The following describes the fields for entering the client's relationship information.
- Relationship Type. Select the client's relationship type for this record from the drop-down list.

- Relationship Person. Select the name for this record from the LOV (search).

- Relationship Org. Select the organization the client is linked to from the LOV (search).
- Is Official Guardian. Select Yes if the name is an official guardian of the client, otherwise leave at No.

- Effective Dates: The Effective Dates have two date fields to enter.
- Date From. If the comm type is current then select today's date (default date in field). If the comm type is not available now but will be in the future, then enter the date it will be available.

- Date To. If the comm type is known to be unavailable in the future, then select the date it will become unavailable. Otherwise leave the default date in the field.
- Red X. Clicking on the Red X (Delete link) will pop a window confirming the action of deleting the record. Clicking on the OK will permanently delete the record.
Attention: See RedX (Delete).
- Save Changes. Click on the Save Changes button to add the records to the form. An additional record will be added to the form every time the form is saved.
