Readmission

If a student plans to apply for readmission to return to DePauw University after withdrawal for medical or mental health reasons, the health provider who treated the student needs to complete the Health Provider Report Form and mail it to the Wellness Center. The appropriate staff member will review this information and make a recommendation to the individual who considers readmission about the student’s readiness to return to school. A completed Authorization for Release of Medical Information should also be attached that allows the health provider to communicate with the Wellness Center staff. Additionally, the student will need to sign a second Authorization for Release of Medical Information so that the Wellness Center can provide information to the individual who considers readmissions.
