Required Student Employment Forms

All students MUST complete the following employment forms before working on-campus.

Both withholding forms (W-4 and WH-4) must be completed and mailed to the Financial Aid Office 204 East Seminary Street; Greencastle, IN 46135  (or send by fax to 765.658.4137)

The Financial Aid Office staff may not give tax advice. Questions or concerns regarding completing tax forms should be directed to your family or to a tax accountant.

1)  Employee’s Withholding Allowance Certificate | Download W-4 

The amount of Income Tax deducted from your pay depends on the information you supply on Form W-4. Determine your eligible exemptions carefully, paying particular attention to the instructions under “Exemption from Withholding” at the top of the W-4 form.

Name: Use your name exactly as it appears on your social security card

Address: Your permanent address where you consider your home to be on a continuing basis.

Exemptions:

* 0 Exemptions: Highest tax taken out = less net pay
* 1 or more Exemptions: Less tax taken out = more net pay
* If exempt is checked - NO Federal tax is taken out. Caution: you may end up owing taxes.

2)  Indiana Employee’s Withholding Exemption Certificate Download WH-4 

Name: Use your name exactly as it appears on your social security card

Address: Your permanent address where you consider your home to be on a continuing basis.

Indiana County of Residence: If you did not live in Indiana on January 1, enter "not applicable".

Indiana County of Principal Employment: If you did not work in Indiana on January 1, enter "not applicable".

Please note that DePauw is located in Putnam County, Indiana.

3)  Employment Eligibility Verification | I-9 Form Available Only On-Campus

Newly hired employees are required to prove their identity and right to work in the United States per Federal regulations. All student employees must appear before a Financial Aid Office staff member and present acceptable forms of identification and proof of employment eligibility at the time employment begins.  New students may complete the I-9 at designated times/locations during orientation or come to the Financial Aid Office (204 East Seminary) after classes begin.

The most commonly used documents are:

  • Driver's License (or School ID w/ Photo) AND
  • Social Security Card (we must see original card) OR
  • Birth Certificate (original or certified copy) OR
  • Passport
