Navigating financial aid requires completing various forms to help determine your eligibility for assistance. These essential documents provide the information needed to assess your financial need and connect you with potential funding opportunities.
Required forms
Free Application for Federal Student Aid (FAFSA) is used to determine your eligibility for federal and state financial aid.
Additional forms
By signing the Family Educational Rights and Privacy Act (FERPA) Form, the student authorizes DePauw Financial Aid staff to release confidential information regarding financial aid and billing to designated individuals (e.g., the student's parents).
Returning students may use the Online FERPA Form.
Current students can submit electronic files to the Financial Aid Office by using our secure upload here.
Register for an account with the Indiana Commission for Higher Education, Division of Student Financial Aid to review the application for state grant assistance and view any outstanding issues/problems.
The Staff Tuition Remission Form allows eligible DePauw employees to apply for tuition benefits for themselves or their dependent children.
The Family Size Verification Form helps the Financial Aid Office accurately assess your family's financial circumstances when determining need-based aid eligibility.
Returning students may use the Online Family Size Verification Form.
The Identity and Statement of Educational Purpose Form verifies your identity and confirms your intent to use federal student aid for educational expenses.
The Asset Verification Form helps the Financial Aid Office accurately assess your family's financial resources when determining need-based aid eligibility.
