Registrar Forms

During the 2021-22 academic year, the Office of the Registrar is transitioning the majority of our forms to Dynamic Forms, an online submission system that collects eSignatures  (i.e., electronic signatures).

  • A system generated timer sets a 45-minute limit for completing online forms.

Forms that are available for online submission are designated in the following as ONLINE. You will need to be logged in to your DePauw Google Account to access these forms.

Student Forms (Frequently Used)

Major Declaration Form (ONLINE)

A separate Major Declaration Form must be submitted for each major you are declaring. Steps for declaring a major:

  • Choose a faculty advisor in the department or interdisciplinary program of the major you are declaring.

  • Meet with your new advisor to review your academic advising transcript. New advisors do not have access to your academic advising transcript until after your Major Declaration Form is processed, so you will need to log in to e-Services in order for you and your advisor to complete this step.

  • Record your advisors first name, last name, and email address because you will need to enter these when completing the online form. Your declaration of major cannot be processed without an advisor's signature.

  • Complete and submit the Major Declaration Form. Upon your eSignature, an email will automatically be sent to your advisor. 

  • After all approvals have been obtained and the Office of the Registrar has processed your request, you will receive a final email informing you that your request has been completed. 

Minor Declaration Form (ONLINE)

A separate Minor Declaration Form must be submitted for each minor you are declaring. Steps for declaring a minor:

  • Choose a faculty advisor in the department or interdisciplinary program of the minor you are declaring.

  • Meet with your new advisor to review your academic advising transcript. New advisors do not have access to your academic advising transcript until after your Minor Declaration Form is processed, so you will need to log in to e-Services in order for you and your advisor to complete this step.

  • Record your advisors first name, last name, and email address because you will need to enter these when completing the online form. Your declaration of minor cannot be processed without an advisor's signature.

  • Complete and submit the Minor Declaration Form. Upon your eSignature, an email will automatically be sent to your advisor. 

  • After all approvals have been obtained and the Office of the Registrar has processed your request, you will receive a final email informing you that your request has been completed.
